Claim Form

Acrobat Reader is Required to download the Claim Form which will be in PDF format. You may already have the Adobe Acrobat Reader on your computer or on disks that came with your computer. If you do, you do not need to download Adobe Acrobat.

Please provide the following information in order to generate a claim form pre-printed with your name and address (NOTE: If you live outside the United States, please leave the City, State, and Zip Code fields below blank). Click "Create Form" in order to view and print your claim form.

ATTENTION:

  • ALL PROOF OF CLAIM FORMS MUST BE PRINTED AND MAILED AS PROVIDED IN THE INSTRUCTIONS IN ORDER TO BE ACCEPTED;

  • When entering your name and address in the fields below, be sure all information is correct and complete prior to clicking "CREATE FORM." Once the "CREATE FORM" button is clicked, you will not be able to make any changes; and

  • Please only click the CREATE FORM button once when generating your proof of claim form.

Once the claim form is printed, complete all relevant fields and sign and mail the completed form to the address listed at the top of page 2 under "Instructions." Be sure to include all supporting documentation with your claim form. Only claim forms with original signatures that are received by mail or overnight or hand delivery on or before the deadline at the address indicated on the claim form (and also seen below) will be accepted. Any proof of claim form submitted by facsimile or e-mail will not be accepted. Once mailed, please allow five to seven business days for receipt and processing of your claim form, and for your claim to be accessible on the Claim Register/Creditor Search tab available on this website.
